The base of the penis is connected to the pubic bone by a suspensory ligament, which restricts its movement and provides a particular angle to the penile base area. During the procedure, a surgeon makes an incision in the pubic area, often in an upside-down V-shape, to access the suspensory ligament. By cutting and releasing this ligament, the internal part of the penis can move forward and outside of the pubic area, effectively extending the visible portion of the penis. Additionally, this release alters the angle of the penile base, further stretching the penis and increasing its proportion outside the body.
