Mr Inder Birdi Explains: Heart surgery can be scary and raise questions about what is wrong with the heart, why it happened, how it can be fixed, and if one can return to their normal life after the procedure. Traditional heart surgery involves making an incision through the breast bone, which takes 12 weeks to heal, preventing heavy lifting and sleeping on one's side. This method is safe and effective, but the time it takes to heal concerns doctors the most. In the last 15 years, the Keyhole Heart Clinic has been performing keyhole heart surgery, which involves small incisions that don't break bones and heal in six or seven days instead of 12 weeks. This method bypasses blocked heart arteries, repairs or replaces valves, closes heart holes, and performs heart rhythm disturbance treatments. The benefits of keyhole incisions include smaller incisions, a more appealing cosmetic appearance, less pain, less bleeding and infection, and earlier discharge from the hospital. The clinic also offers treatments without incisions for the right patients, which involve passing stents or repairing or replacing heart valves through wires in the groin. Inder Birdi Heart surgeon explains Transcatheter Aortic Valve Implantation (TAVI) as a treatment for Aortic valve stenosis compared to traditional surgery. Here's a summary: Introduction to Aortic Valve Stenosis: Explains the role of the aortic valve in blood circulation. Describes how thickening and calcification of the valve can lead to aortic valve stenosis. TAVI Overview: Highlights TAVI as a promising treatment, particularly for older patients. Acknowledges challenges, such as limited long-term data on TAVI valve durability and potential issues with calcified valves. Concerns with TAVI: Discusses the risk of poor sealing around the TAVI valve, especially with mild or moderate leaks. This raises the issue of calcium displacement during TAVI, potentially causing damage to the heart's conduction system and increasing the risk of needing a pacemaker. Traditional Surgery Advantages: Presents aortic valve replacement surgery as the mainstay with reliability, predictable durability, and low pacemaker risk. Explains the need for a balance between surgery and TAVI based on age, health, and long-term outcome considerations. Balancing Treatment Options: Discusses factors influencing the choice between surgery and TAVI, emphasising the need for a personalised approach. Highlights the durability of newer-style pig and cow valves, making TAVI a potential option for future valve replacement. Guidelines and Decision-Making: Mentions general guidelines suggesting surgery under 65 and TAVI over 80 but emphasises the importance of individual patient considerations. Explains the collaborative decision-making process involving a multidisciplinary team of cardiologists and surgeons. In summary, the video provides valuable insights into the pros and cons of TAVI and traditional surgery, emphasising the need for a tailored approach based on individual patient factors and preferences. Meet our patient with aortic stenosis, who underwent total endoscopic minithoracotomy aortic valve replacement, a minimally invasive surgical procedure performed through a small incision between the 2nd and 3rd rib without any bone division or rib spreading. Due to the expertise of our surgeons, the procedure was performed on a television screen, significantly reducing his pain and discomfort after the procedure, and ensuring more rapid healing. Just two months after surgery, he sent us this amazing video of him performing a complex judo routine, which speaks volumes about the effectiveness of the surgery. Had he undergone breastbone surgery, he would have been unable to perform this routine for 4-6 months after surgery, but thanks to our innovative and minimally invasive approach, He was able to resume his active lifestyle in record time. We're incredibly proud of our wonderful patient and his amazing recovery, and we're committed to providing all of our patients with the same level of care and expertise that made his remarkable journey possible.
