What is septorhinoplasty?
Autore:The nose is the most prominent feature on the middle third of face. The size and shape of nose affects the harmony between upper and lower third of face.
The nasal form and function both are equally important for the sense of well -being in any individual. The form and function are affected by the ethnic origin, genetics and trauma.
Accordingly, there are three procedures possible to alter the form and function i.e. A) Septoplasty: to correct the septal deviation and improve the breathing. B) Rhinoplasty: Which alters the nasal form. C) Septorhinoplasty.
Septorhinoplasty is a surgical procedure that combines two distinct elements: the correction of the nasal septum and the reshaping of the external structure of the nose. It is performed to improve both functionality and aesthetics. For patients considering this surgery, it is important to understand how it works, the benefits it offers, and what to expect during and after the procedure.
Leading consultant plastic surgeon Mr Makarand Tare explains all you need to know about the procedure.
Who is suitable for septorhinoplasty?
Septorhinoplasty may be recommended for individuals experiencing breathing difficulties due to a deviated septum, a structural issue within the nasal cavity that can obstruct airflow. It is also sought by patients who are unhappy with the appearance of their nose, whether due to congenital factors, injury, or previous surgeries. If you are considering this procedure, a thorough consultation with a specialist will be necessary to assess whether you are suitable for surgery.
What does septorhinoplasty involve?
Septorhinoplasty is a complex procedure, so you should always consult the surgeon who has reasonable experience and understanding of both the nasal form and function. The procedure typically involves two main components: correcting the nasal septum and reshaping the external nose.
Septorhinoplasty = Septoplasty + Rhinoplasty.
Septoplasty: Correction of nasal septum refers to straightening / resection of the internal midline cartilage and bone, which can improve airflow and reduce the nasal obstruction.
Rhinoplasty: External reshaping involves altering the shape and size of the nose to achieve a more balanced appearance. This may involve reducing or increasing the height, length, projection of nose, alteration of tip configuration, reducing the nasal hump, narrowing the nostrils, or refining certain areas such as the tip or bony bridge with bone remodelling.
The procedure is done under general anaesthesia and takes around 3-4 hours depending on the complexity of the case. You will have internal and external nasal splints with a nasal pack for one night. The nasal pack will be removed the next day a couple of hours before the discharge. You are supposed to sleep in 45 -degree angle with head end up position.
You will be seen at weekly intervals for removal of splint and sutures. The bruising will settle down in a couple of weeks but the swelling will take almost a year to settle down fully.
What are the benefits of septorhinoplasty?
The benefits of septorhinoplasty extend beyond aesthetic improvements. Firstly, correcting a deviated septum can significantly enhance breathing, reducing issues such as nasal congestion, snoring, and difficulty breathing during physical activity. Secondly, patients often experience a boost in self-confidence following the surgery, with a more proportionate and attractive nasal appearance. Additionally, septorhinoplasty can address functional problems and cosmetic concerns simultaneously, making it a comprehensive option for patients.
What can you expect during recovery?
Recovery from septorhinoplasty typically takes several weeks. Immediately after surgery, you may experience some swelling, bruising, and mild discomfort. Your surgeon will provide specific post-operative care instructions, including how to manage swelling and ensure proper healing. It is common for patients to have nasal packing or splints placed inside the nose for a few days post-surgery to stabilise the internal structure. Most individuals can return to work or school after two weeks, although strenuous activities should be avoided for eight weeks.
Are there any risks or complications?
It is very important to have realistic expectations. It is not always possible to achieve a perfect nose due to previous trauma/ surgery, internal scarring, cartilage and soft tissue memory.
Like all surgical procedures, septorhinoplasty carries some risks. These may include bleeding, infection, scarring, numbness of nose, septal perforation, undercorrection, recurrence of the deformity and the need for revision surgery.
There is a possibility of revision surgery ( 25%) if the desired outcome is not achieved, or if complications such as nasal asymmetry occur. However, with a skilled and experienced surgeon, complications are minimised.
Ultimately, septorhinoplasty is a tailored procedure, and a thorough consultation will help patients understand the best course of action for their individual needs and goals.
