Pain in the big toe can be a sign of gout, injury or hallux rigidus but also ill-fitting shoes! In this video Consultant Orthopaedic Foot and Ankle Surgeon, Mr Andrew Goldberg, explores the causes of pain in the big toe. The commonest cause is hallux rigidus which translates into “Stiff big toe”. Just like any joint, the end of the bone surfaces are covered in shock-absorbing cartilage similar to the gristle at the end of a chicken drumstick. In arthritis, the cartilage wears away and bone rubs against bone, which causes pain. The big toe joint is a particularly complicated joint because there are two pea-shaped bones sitting under the big toe called the sesamoid bones which are also involved in motion. Hallux rigidus can affect all individuals but is more likely to affect those who are active and regularly participate in sporting activities or those with inflammatory conditions such as gout or rheumatoid arthritis. It is well known that hallux rigidus may begin early in life, even in the teens. Ankle arthritis can affect your quality of life as much as end stage hip arthritis and heart failure. In this article, Mr Andrew Goldberg, Consultant Orthopaedic Foot & ankle Surgeon at the Wellington Hospital in London, UK, explores ankle arthritis and osteoarthritis. In ankle arthritis, the cartilage is worn and the bone starts to rub on the bone. Resulting in ankle joint pain. As with other joints, articular cartilage lines the bones surface. This is a form of shock absorber, allowing smooth gliding motion during walking and running. The normal smooth motion of the joint is affected when this cartilage becomes worn. This can happen as a result of wear and tear or damage following injury or disease. The rubbing of the bone on bone causes pain and stiffness and these are the main symptoms of ankle arthritis. People with ankle arthritis often also limp and find it impossible to participate in sports or even normal walking without pain. An ankle fracture is a common injury that refers to a fracture in one or more of these bones as a result of a fall or impact. The fracture can sometimes result in dislocation of the ankle too. Total ankle replacement and ankle fusion are both successful operations to treat ankle arthritis. In this video, Mr Andrew Goldberg, an orthopaedic foot and ankle surgeon at the Wellington Hospital in London, offers advice and guidance to explain the pros and cons of both procedures as well as top tips to ensure that you get the best outcome. ANKLE REPLACEMENT VS ANKLE FUSION Most ankle arthritis can be treated without surgery. Non-surgical treatments include activity modification, physical therapy, weight loss, supportive boots or ankle braces and pain killers. Once non-surgical treatments have been exhausted then you may consider surgery. There are two recognised treatments for ankle arthritis. Both an ankle replacement and an ankle fusion can improve your quality of life. ANKLE FUSION Because the ankle and foot have about 30 mobile joints, fusing just the ankle doesn't mean you would walk with a peg leg, far from it. The other 29 or so joints compensate and so if you have healthy adjacent joints, then there is still quite a bit of motion. So in patients with healthy adjacent joints, you may walk normally and it might be very difficult for someone to know you have had an ankle fusion. However, because the ankle joint itself is now stiff then of course the other joints around it have to take on the extra load and they can, with time wear away and become arthritic and hence painful. TOTAL ANKLE REPLACEMENT Many GPs and even some specialists don't know that replacing an ankle is even possible. Fortunately, advances in technology plus our advanced understanding of how the ankle complex works, means that it is now very possible to replace the human ankle joint. An ankle replacement is where the ankle joint is resurfaced with metal implants and a plastic liner and because it maintains motion, theoretically lowers the risk of adjacent joint arthritis, but this remains to be proven. New scientific research using a standing CT scan has allowed scientists to see how the foot looks and behaves in a functional standing position inside shoes such as high heels.
